Ambulatory Care Pharmacist – Cardiology SUNY Upstate Medical University is currently seeking an Ambulatory Care Pharmacist to provide clinical pharmacy services for the hospital’s cardiology clinic. This pharmacist will operate under the ambulatory care pharmacy services umbrella of the hospital’s pharmacy department. This pharmacist will work directly with providers and support staff to optimize medication therapy, provide patient/provider/nurse education, and assist with transitions of care management.
