POSITION SUMMARY:
The Cardiac & Stroke Program Coordinator provides support for Emergency Department, Interventional Cath Lab, In-patient Care Departments and overall regional program quality and growth.
The Cardiac/Stroke Program Coordinator will:
Coordinate and facilitate the regional Cardiac and Stroke program in collaboration with SJRMC internal and external stakeholders.Â
Ensure the quality and growth of the Cardiac and Stroke program through robust QI, QA, and process improvement activities.Â
Supports the development of relationships between the medical center and other health care organizations, locally and regionally. Â Â Develops contacts and appropriate processes to facilitate and maintain working arrangements that create opportunities for the delivery and coordination of care in the service area.
Identifies opportunities to improve service delivery on an ongoing basis and reports to the respective director. Â
PRIMARY DUTIES:
Provide for overall guidance of the Cardiac & Stroke program growth and development.Â
Keeps abreast current evidence-based guidelines and practices related to this program. Responsible for sharing of this knowledge and information to applicable parties.Â
Maintains Quality Improvement (QI) and Quality Assurance (QA) activities for programs with integration into the Medical Center’s committees and quality reporting structure.
Regularly track and report on key quality indicators for programs and use indicators to drive program improvements in collaboration with nursing, ancillary, and medical staff.Â
Meet periodically with referral physicians, hospitals and other healthcare partners. Travel as needed to regional facilities to establish relationships and provide support in utilizing the process.
Assists with concurrent abstraction of cardiac and stroke charts utilizing designated data bases.
Performs Cardiac and Stroke performance improvement audits.
Participates in Emergency Services, Cardiovascular Quality and Critical Care Committees.Â
Develop and foster collaborative relationships with all hospital departments to facilitate and support quality program care.
Participate in state and regional TSE care activities.
Perform rounds on cardiac and stroke patients as needed.
Provide education to SJRMC direct care staff to meet the needs of each patient while utilizing evidence based care modalities.
Assist with/present community wide program specific education.
Any additional duties as assigned by Department Director.Â

Qualifications
Required:
Associate of Science degree in nursing (ASN)
Current license in the state of Idaho, or licensure eligibleÂ
BLS on first day or to be assigned by the Learning Center
Requires proficient use and knowledge of PC applications (EHR, WORD, EXCEL) and the ability to operate a computer terminal to access and make entries in patient account records; ability to review/audit/abstract medical records
Ability to communicate effectively both orally and in writing with respect, tact and persuasiveness.
Strong time management and organizational skills and the ability to cope with stressful situations with a positive attitude and excellent customer service.
Preferred:
3-5 years' experience in an acute care setting (specific areas to include ER and/or Critical Care)
