This position is a Cardiovascular Technologist who has demonstrated advanced training in Interventional Cardiac Catheterization procedures; Electrophysiology and mapping procedures; and procedures performed in a Hybrid OR Suite. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. English Language Proficiency: Medical Instrument Technicians must be proficient in spoken and written English in accordance with chapter 2, section D, paragraph 5a, this part. Certification: Registration by Cardiovascular Credentialing International (CCI) as a registered Cardiovascular Invasive Specialist (RCIS) is highly desirable. Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) certifications are also desirable for employees at this level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Medical Instrument Technician ( Cardiovascular Cath) GS-8 (a) Experience. At least 1 year of experience comparable to the next lower grade level (GS-8), which is directly related to the duties of the position to be filled and must fully meet the KSAs at that level. This would be experience which demonstrates possession of the knowledge required in order to provide all aspects of cardiac catheterization technician services. In addition, the candidate must demonstrate the following technical KSAs and the potential to acquire the assignment specific KSAs : Do you demonstrate the following GS-08 Knowledge, Skills, and Abilities: 1. Ability to scrub, circulate, and monitor advanced procedures (including electrophysiological) to include rotoblation, intravascular ultrasound, RADI or wave-wire, angiojet, filter wires, myocardial biopsies EPS, permanent pacemaker insertions, AICD insertions, and biventricular pacemaker insertion. 2. Advanced knowledge of equipment, medications, and supplies used in the Cardiac Catheterization Laboratory and the ability to set-up, calibrate, operate, and troubleshoot this equipment. 3. Knowledge and understanding of wave forms from the great vessels and all chambers of the heart including the ability to recognize and distinguish normal from abnormal, artifact from abnormality, and forewarn the physician of an impending life-threatening situation. 4. Knowledge of quality control and performance improvement indicators for the Cardiac Catheterization Laboratory and ability to prepare and analyze quality control of test results. Major Duties:\nThe incumbent is skilled to assist with diagnostic and advanced special procedure radiographic procedures in cardiac catheterization, electrophysiology and hybrid suites. The incumbent is skilled and experienced in performing the following list of procedures that are performed: Cardiac Catheterization Lab: Percutaneous coronary interventions, Coronary stent placements Coronary atherectomy, coronary flow evaluation, Valvuloplasty, Swan ganz catheter placement Pericardiocentesis Temporary Pacemakers (highly acute patients) CTO: Chronic Total Obstruction procedures Advanced knowledge of fluoroscopic imaging and the use of equipment Intravascular coronary ultrasound, ventriculography The incumbent must also have experience with the following specialized equipment for procedures that are performed in the cardiac catheterization lab: Specialized Equipment Competency: ACIST Injector IABP: intra-aortic balloon pump Phillips Xcelera recording system TVC Imaging: (True Vessel Characterization) IVUS: intravascular ultrasound Rotoblator Zoll Defibrillator Med-Rad injector Volcano OCT: Optical Coherence Tomography Electrophysiology Lab Procedures: Radiofrequency ablation, endomyocardial biopsy Temporary and permanent pacemaker implantation, Bi-ventricular ICD placements Subcutaneous loop recorder implantation Tilt table testing, trans esophageal testing, and other specialized testing Left ventricular assist device placement (LVAD), renal carotid, iliac, visceral and peripheral angiography and intervention, venograms, PICC line placement. Subcutaneous ICD Implantation The incumbent must also have experience with the following specialized equipment for procedures that are performed in the electrophysiology lab: Specialized Equipment Competency: Cath lab recording and archive system Intracardiac ECHO Catheter & Interface Module Electro-cautery Pulsar II Generator EP mapping and recording systems Cryoablation Cardioversion Defibrillation Other Specialized Equipment Competency\nThis position requires the Cardiovascular Technologist (CVT) have advanced training and knowledge with the use of the X-Ray suite, utilizing specialized equipment to perform a variety of diagnostic and interventional angiographic procedures, complex vascular and nonvascular interventional and therapeutic procedures. The CVT must be knowledgeable in systems that involve radiological archiving and communication. The CVT must be skilled in sterile technique and operative standards, contrast administration, and skilled in the use of power injectors. The CVT must have knowledge and demonstrate sterile technique and adhere to standards for invasive procedures. The incumbent must also have advanced training and knowledge of specialized procedures and or equipment in the following: Filter systems for coronary interventions saphenous vein grafts: SVGs. The incumbent can set up a sterile cath/electrophysiology table, manifold (including flushing air out and flushing solution/contrast to appropriate lines), and preparation pre-procedure of patients, operation and maintenance and recording of hemodynamic data using a specialized multichannel, multifunctional electronic recorder. The duties in this position include coordinating, supervising, as well as operating and monitoring diagnostic/interventional instruments and equipment associated with cardiac procedures. Physiological measurements in the Cardiac Catheterization/Electrophysiology Suite such as cardiac pressure transducers, thermodilutional cardiac output computers, blood gas oximeters, and oxygen consumption computers.
