The Nurse Practitioner (NP II) is a licensed practitioner who isresponsible for the management and coordination of care for the Cardiothoracic-VascularSurgical and Cardiovascular patients in accordance with State and Federal rulesand regulation and the standards of practice for NP's (BRN, 2013; AANP, 2019).The NP practices as a member of the interdisciplinary healthcare team usingstandardized procedures to independently examine, diagnose, treat, implement,and evaluate patients in collaboration with supervising physician(s).
The incumbent supports management in achieving a high-qualityprogram for the Cardiothoracic-Vascular Surgical and Cardiovascular patientpopulations, for both clinical outcomes and customer service in an efficientmanner. The NP II is knowledgeable of the latest clinical trends, bestpractices, regulatory requirements and reimbursement patterns for patients inthe specialty program.
Expert ClinicalPractice and Collaboration
Independently examines, diagnoses, orders diagnostic studies, and developstreatment plans for Cardiothoracic-Vascular surgery and Cardiovascular patientsusing approved standardized procedures.
Prescribes appropriate pharmacologic and non-pharmacologic treatmentmodalities
Evaluates the effectiveness of interventions and adjusts treatment asindicated.
Provides direct and indirect patientcare.
Coordinates, prioritizes and organizes individualized patient care incollaboration with supervising physician(s) and multidisciplinary team
Performs ageappropriate care for complex acute, critical, and chronically ill patients.
Collaborates with members of the healthcare team to create a patientplan of care.
Transcribesdaily notes in patients' chart who are assigned to service, documenting updatedplan of care in accordance with supervising physician and multidisciplinaryteam
Provides individualized patient and family education and documents inthe patient teaching plan.
Communicates with supervising physician and physician consultantstreating the patient.
Anticipates discharge on admission and plans accordingly.
Ensures accurate patient problem list and medication reconciliation onadmission and discharge.
Utilizes culturally specific competencies for diverse patients.
Utilizes agespecific competencies for adult-gerontologic patients.
Ensures accuracy of acuity with appropriate documentation.
Ensures a seamless transition through the hospital continuum of care.May follow in an outpatient clinic or physician's office.
The Cardiothoracic-Vascular Surgical and Cardiovascular NP manages thecare of the patient through the healthcare continuum.
May practice in a clinic, inpatient setting and/or outpatient setting.
May oversee support groups for specific patient diagnosis or populationrelated to cardiothoracic/cardiovascular surgery.
Identifies ideas and insights into clinical improvements for specialtypatient population.
Ensure that queries generated by medical records regarding the accuracyof documentation have an appropriate response.
Monitor and manage the patient progression through hospitalization.
May oversee the accurate collection and entry of data specific topatient population. May prepare or oversee the preparation of reports regardingspecialty care.
Reviews clinical outcomes, progression of care, and financial indicatorswith appropriate Medical Directors and other specialty physicians.
Provide education for members of the interprofessional team regardingspecial needs in the care of specialty patient population.
Acts as a consultant to other care lines, service lines, BPTs onspecialty specific patient care needs.
Actively participates in requested committees and meetings.
Quality Improvementand Research
Recommends, supports, participates or leads quality improvementinitiatives regarding specialty care.
May participate as an investigator, co-investigator in research studiesspecific to patient population.
Participates in requested peer review activities.
Assists with teaching classes thatpertain to specialty patient population
Complies with all policies and procedures.
Completesthe 'Clinical Competency Assessment and Proctoring Plan' within allotted timeframe from hire date.
Maintains standardized procedures and privileging/credentialingrequirements according to medical staff AHP rules and regulations.
Participates in the orientation and development of students and/or newstaff as assigned.
